Use regedit.exe to delete the following additional values from the Windows Registry:

• HKEY_CLASSES_ROOT\Local Settings\Software\Microsoft\Windows\Shell\MuiCache\D:\PCSX2\pcsx2-qt.exe.FriendlyAppName
